Phase II: Outpatient Cardiac Rehabilitation

Patients are encouraged to participate in our Phase II Outpatient Cardiac Program following hospital discharge or a cardiac diagnosis. Phase II cardiac rehabilitation generally consists of a 12 week-program that strives to assist patients in:

  • Increasing exercise tolerance
  • Reducing symptoms of angina, dyspnea, and fatigue
  • Improving blood lipid and blood sugar levels
  • Reducing weight and stress
  • Improving psychosocial well-being and reducing cardiac mortality
  • Modifying diet and making healthier choices
  • Smoking cessation

This 12-week program includes:

36 one-hour-long sessions (3x/week) of a physician directed program that includes ECG telemetry monitoring, aerobic exercise to promote endurance, resistance training to improve musculoskeletal strength, and flexibility while teaching patients to assess their tolerance for exercise by developing self-monitoring skills. In addition, the program provides educational seminars on important aspects of cardiac well-being as well as nutritional guidance in adopting a heart-healthy diet.

Phase III: Outpatient Maintenance Program

Once a patient has completed their 36 rehabilitation sessions, they have the option to participate in a Phase III exercise maintenance program at Montefiore Nyack Hospital. This is a self-pay program that enables the patient to continue to use our exercise facility in order to maintain the benefits derived from their Phase II program while continuing to receive the guidance and supervision of our clinical staff.
