As a growing number of patients are discharged from the hospital sooner, with more complex medical needs, the role of home care is expanding. Today, home care nurses care for very sick patients who require extremely skilled high-tech nursing care at home.

Home care organizations also provide physical therapy, occupational therapy and speech therapy, as well as social workers and home health aides.

In many cases, home care is the right choice for a patient who needs extra help after they leave the hospital. Patients can recover in the comfort and privacy of their own home and are less likely to develop infections at home.

Individualized Care

Montefiore Nyack Hospital Home Care provides an individualized care plan for each patient. Our team visits the patient at home and evaluates what needs to be done to keep the patient safe. For instance, they may need adaptive equipment in the bathroom so they are not at risk of falling, or a cane to get up and down the stairs. They may need help figuring out how to manage the steps outside their home to get to the doctor’s office. After evaluating the patient at home, the nurse might recommend to the physician that the patient may need physical or occupational therapy. Our goal is to get patients to be as independent and functional at home as they can be.

Home care isn’t just about caring for the patient. We also work with families to teach them how to care for themselves while caring for their loved ones.

Reducing the Risk of Re-Hospitalization

Another benefit to home care is that it can help reduce the risk of re-hospitalization. Patients can become confused about their medicines, particularly if their medications were changed in the hospital. The home care nurse works closely with the patient’s doctor to review all medications—both prescription and over-the-counter—and compares the patient’s medications at discharge with what medications the patient has at home. The nurse makes sure the patient knows when and how to take their medication and what the side effects are. We become the eyes and ears of the physician in the patient’s home. This is very helpful to patients and families who have a difficult time getting to the doctor’s office.

Home care is a very supportive, nurturing environment with a tremendous amount of teamwork among the staff and physicians.

We always have a nurse on call 24 hours a day who can evaluate the patient’s needs over the phone and visit the patient’s home if needed. This helps to avoid unnecessary emergency room visits.

For many of our patients, home care is paid for by Medicare. We also accept Medicaid and most insurance plans.

The rating, between 1 to 5 stars, is calculated using each home health agency’s scores on seven individual quality measures, which are based on home health agency patient assessments and Medicare claims. A 4.5-star rating means the agency performed better than other agencies on seven measured care practices and outcomes.
