Develops, implements, evaluates and updates nutrition education programs for the Rockland County Community, including weight loss and diabetes prevention and management training, specifically targeted to underserved populations, making sure that the programs meet age and cultural needs of audiences. Provides Medical Nutrition Therapy based on the Nutrition Care Manual to ambulatory patients including management of obesity, diabetes, heart disease, multiple sclerosis, cancer, and all other nutrition related diseases. Collaborates with appropriate hospital staff, the local health department, and community members and organizations, in the development of programs and nutrition therapy of patients.
2. Experience: Health education in a community setting. Strong knowledge of nutrition therapy, diabetes, adult learning, and change theories.
3. Training/Certification: CDES preferred, EPIC knowledge preferred.
4. Licensure: NYS RDN License
5. Other: Strong computer skills, Bilingual Skills in Spanish preferred, knowledgeable in data collection & outcomes measurement. Excellent public speaking and strong interpersonal skills are necessary as they are an integral aspect of this position. Outgoing, professional demeanor.
