Release of Medical Records
To request a complete copy of your medical record or radiology reports and images, or to send a copy to a third party, please download and complete the Authorization for Release of Health Information form. You may return the completed form to the Medical Records Department by Fax at 845-348-8433 or by email at ROI@montefiorenyack.org. We can provide copies of your medical records via secure email, and/or we can mail a CD to you.
Requests from attorneys can be mailed to: Medical Records Dept., Montefiore Nyack Hospital, 160 North Midland Avenue, Nyack, NY 10960.
There is a fee of .75 cents per page for copies of medical records. For additional information about the release of medical records, call 845-348-2527.
Birth Certificate Inquiries
To request a birth certificate for an infant 3 months of age or younger, please call 845-348-2885. For all other birth-certificate requests, please contact the Village of Nyack:
