Your health is our mission; that’s why the Montefiore Nyack Hospital Community Health & Wellness Department is proud to offer complimentary health and well-being education events and programs to Rockland County residents.

Health Education 

Our approach to health education is rooted in a collective commitment to care. Our team of knowledgeable experts partners with corporations, community groups, schools, congregations, and faith-based organizations throughout Rockland County to deliver the latest information on health and well-being. Through presentations, workshops, and exhibitor booths, we educate the public on a variety of important health topics that help prevent disease and promote healthy habits for people of all ages.

Our free Speakers Bureau offers expert-led health education programs delivered by physicians and healthcare professionals, with topics tailored to your organization’s needs and interests.

Community Resources

Health Screenings 

Montefiore Nyack Hospital brings screening services to community groups and local organizations to assess adults for a variety of diseases and conditions.  

We offer free health screenings for diabetes, high blood pressure, obesity, cholesterol, and HIV/STI. We also assess cardiovascular disease risk and stroke risk. These services include complimentary health education and nutrition advice. Health screenings need to be reserved for 2 months in advance. In addition, we offer free cancer screenings for uninsured and underinsured individuals at the hospital several times per year. All participants receive a same-day complimentary nutrition visit with a registered dietitian.  

Eligibility is determined by the NYS Cancer Services Program. Call 845-348-8511 to learn about screening dates, eligibility, and registration.

Community Chats 

Lunch & Learn! Join us on Thursdays from 12:00 p.m.–1:00 p.m. for free, live webinars with a variety of subject matter experts. Community Chats is designed to provide insight into a variety of health conditions, wellness topics, and resources available in the community—all explained in clear terms, comprehensible to all. While maintaining their anonymity, participants can ask questions and share their experiences related to each session’s topic. While sessions are conducted in English, closed captions are available in multiple languages.

Diabetes Programs 

Our Community Health & Wellness Department’s signature diabetes programs, offered in English and Spanish, help patients take control of their health. From weight-loss support to groups that remind you you’re not alone, we offer a range of opportunities to manage diabetes and prediabetes.  

Diabetes Prevention Program 

You don’t have to carry the weight of diabetes. Research has shown that those at high risk for Type 2 diabetes who lose a modest amount of weight and get at least 150 minutes of physical activity per week can reduce their risk of getting Type 2 diabetes in half.  

Our Diabetes Prevention Program is a free, weekly program for anyone who has been diagnosed with prediabetes, has a history of gestational diabetes, or is at high risk of getting Type 2 diabetes. Through the guidance of a personal health coach, this program helps you achieve your desired weight, reach your physical activity goals, and prevent or delay the onset of Type 2 diabetes.

Outpatient Diabetes Self-Management Program 

You can take control of your diabetes. Accredited by the Association of Diabetes Care and Education Specialists, our 4-week program empowers people living with Type 1 and Type 2 diabetes to control their blood sugars, avoid complications, reduce hospital visits, and live healthier.

Diabetes Support Group 

Looking for a more flexible program? Our virtual diabetes support group offers a safe space to ask questions about diabetes, learn from our experts, and share your experience with others who are living with diabetes or prediabetes. Meetings are held once per month, either by phone or via Zoom. You may attend this group as often as you like. The most important takeaway is that you are not alone in your diabetes journey. We are here to support you when you need it most.

Mamás Maravillosas 

Mamás Maravillosas es un programa gratuito de prevención de diabetes y pérdida de peso diseñado para todas las madres de habla hispana. Mamás Maravillosas ayuda a las participantes a perder peso, mejorar la salud en general, aprender hábitos saludables para toda su familia y reducir el riesgo de complicaciones durante embarazos posteriores.

Community Health Needs Assessment and Service Plan

Community Event PhotoMontefiore Nyack Hospital is committed to improving the health and well-being of the community. In line with the New York State Commissioner of Health's mandate, the Hospital, along with our community partners and the Hospital's Community Service Plan (CSP) Committee, regularly reviews current initiatives, strategic plans, and prevention priorities.

The CSP Committee's mission is to assess our community's health needs and recommend programs and services to meet them. In addition, the Hospital conducts a community health needs assessment (CHNA) every 3 years, which also guides the development and delivery of the Hospital's programs and services.

Our action plan has been identified through our committee as we continue our ongoing dialogue with patients and their family members, local organizations, area business leaders, elected officials, not-for-profit corporations, and the Rockland County Department of Health.
