July 2, 2025 – Montefiore Nyack Hospital is proud to reaffirm its commitment to becoming an Age-Friendly Health System, joining a growing number of healthcare organizations across the United States dedicated to providing age-sensitive, high-quality care for older adults. According to a report from S&P Global, one in five Americans is expected to be 65 years old or older by 2030 as the U.S. population continues to age. Currently, the population aged 65 and older is estimated at 61.2 million, representing approximately 18.6% of the total U.S. population. This number is expected to grow, with projections indicating that more than 71 million individuals will be in this age group by 2030.
"As the population ages, it is essential that our hospital evolves to meet the unique needs of older adults,” says Anthony Matejicka, DO, Vice President and Chief Medical Officer at Montefiore Nyack Hospital. “Becoming an Age-Friendly Health System ensures we are providing safe, effective, and compassionate care that respects the values and preferences of our senior patients."
This national movement, led by The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the American Hospital Association and the Catholic Health Association of the United States, is helping healthcare organizations implement a set of evidence-based interventions specifically designed to improve care for older adults. Collaborative learning opportunities across the movement bring together healthcare teams committed to sharing data and improving outcomes. All teams strive toward implementing age-friendly best practices across hospitals, clinics, nursing homes, home health agencies, and primary and specialty care settings.
Montefiore Nyack Hospital now joins an international group of more than 5,200 health systems working to tailor care to patients’ goals and preferences and to deliver care of the highest quality. The goal is to ensure that every older adult receives the best possible care by focusing on what matters most to them.
The Age-Friendly Health Systems initiative is based on the 4Ms Framework, which guides care for older adults across all settings:
• What Matters – Knowing and aligning care with each older adult’s specific goals and care preferences.
• Medication – Using age-appropriate medications and avoiding those that can interfere with what matters or cause harm.
• Mentation – Preventing, identifying, treating, and managing dementia, depression, and delirium.
• Mobility – Ensuring older adults move safely every day to maintain function and independence.
